By accessing the provider network information, the member acknowledges their understanding that co-payments and allowances may differ when a member selects a Wal-Mart Optical, Sam's Club Optical, or Out-of-Network location.  Additionally, the member understands that while the Wal-Mart and Sam's Club optical shops are participating providers, the doctors at these locations are independent contractors and may not be "In-Network" providers.
