To access your BaltasVision, LLC benefit:
- Choose from any of the BaltasVision, LLC network optometrists, ophthalmologists, or optical retail providers.
- Schedule an appointment. When scheduling, please mention that you are a BaltasVision, LLC member. Many providers also accept walk-in visits.
- When you arrive, identify yourself as a BaltasVision, LLC member and present your ID card.
- The BaltasVision, LLC provider will take care of the rest!
Your plan provides you with many options to locate providers and review your benefit levels. Members can access the other portions of the website, refer to their plan documents or contact Member Services (270) 769-1336 for both benefit and provider information.
